Find your Insurance Card and ID

Call (510) 278-9702 for an appointment. Tell the receptionist how you heard about us and that you have Insurance that may cover Chiropractic.

During the call the receptionist may ask about your name, your birthday, insurance name, and insurance ID number.

Bring your Insurance Card/s and ID with you to your appointment and we'll see you then . . . .

C. If your are Medicare Patient . . . .

Find your Medicare Card and ID

Call (510) 278-9702 for an appointment. Tell the receptionist how you heard about us and that you have Medicare and (if you have) Second Insurance that may cover chiropractic.

During the call the receptionist may ask about your name, your birthday, Medicare ID number, and (if you have) second insurance name and second insurance ID number.

Bring your Medicare Card, ID, and (if you have) Second Insurance Card to your appointment and we'll see you then . . . .

D. If your are Work’s Compensation Patient . . . .

During the call the receptionist may ask about your name, where you work, your work's insurance, and how long ago was the accident.

Bring any papers you have from your work, insurance, attorney if you have one (not needed thou), from any other doctors (if any) to your appointment and we'll see you then . . . .

E. If your are Auto Accident Patient . . . .

During the call the receptionist may ask about your name, , your auto insurance company, your ID number, your claim number, and attorney information if you have one (not needed thou).
